FSFH End of Year Function Door Prize Terms and Conditions

  1. Information on how to enter this raffle competition forms part of these terms and conditions of entry. Entry into this Raffle is deemed to be acceptance of these terms and conditions.
  2. The raffle will be drawn during the FSFH End of Year Function held on 1st December 2023 at Moore & Moore Café, 46 Henry Street Fremantle.
  3. The raffle is being run by the MDA National Insurance Pty Ltd (MDA National Insurance) ABN 56 058 271 417 AFS Licence Number 238073 of Level 3, 88 Colin Street, West Perth WA 6005, Australia, phone 1800 011 255. MDA National Insurance is a wholly owned subsidiary of MDA National Limited ABN 67 055 801 771 (‘MDA National”) (together the ‘MDA National Group’).
  4. Entry is open to all Doctors attending the FSFH End of Year Function held on 1st December 2023 at Moore & Moore Café, 46 Henry Street Fremantle.
  5. The giveaway is not open to employees of the MDA National Group, their immediate families, and affiliated other parties.
  6. The entry forms to enter the raffle will be available at the link below between 8am Saturday 11th March 2023 and Friday 1st December at 12noon. This link will be included on event tickets, invitation & event homepage.
  7. The prize consists of ONE set of Bose Quiet Comfort Earbuds valued at $400.
  8. The raffle will be drawn at FSFH End of Year Function held on 1st December 2023 at Moore & Moore Café, 46 Henry Street Fremantle by Dr David Hoppe.
  9. The result is final, and no correspondence will be entered into.
  10. The raffle winner must, on request by MDA National Insurance, be able to provide evidence which, to the reasonable satisfaction of MDA National Insurance, demonstrates that the winner is an eligible entrant and has complied with these Terms and Conditions.
  11. Should the raffle winner not be present at the event then a re-draw will occur.
  12. Redraws will follow the same process until a successful claim is made.
  13. The raffle prize will be deemed unclaimed if MDA National’s reasonable attempts to draw the lucky winner is unsuccessful.
  14. The raffle prize must be taken as stated and no compensation will be payable if the raffle winner is unable to use the prize as stated. The prize is not transferable to another person (unless agreed by MDA National Insurance) or exchangeable for other goods and services and cannot be redeemed as cash or other than as stated in the voucher.
  15. The MDA National Group and its respective officers and employees will not be liable for any loss, damage or personal injury whatsoever (including but not limited to direct, indirect or consequential loss) which is suffered or sustained in connection with this raffle, the promotion of this raffle or the use of any prize, except for any liability which cannot be excluded by law. All entrants indemnify the MDA National Group against all liability, cost, loss or expense arising out of their participation in the raffle.
  16. The MDA National Group is not responsible or liable for any late, lost, incorrectly submitted, delayed, ineligible, incomplete or misdirected entry whether due to error or otherwise or for any entrant’s inability to participate in the raffle.
  17. Entry details remain the property of the MDA National Group. The name of the raffle winner may be used for promotional purposes by the MDA National Group, unless the raffle winner otherwise notifies the MDA National Group at the time of accepting his/her prize. Eligible entrants consent to MDA National Group using the contact details provided in connection with their MDA National Insurance policy for the purposes of facilitating the conduct of the raffle and awarding any prizes.
  18. The MDA National Group collects, uses and discloses personal information of entrants in accordance with its Privacy Policy for the purposes of conducting this raffle. Entrants may access, change and/or update their personal information by contacting the MDA National Group on the number below.   19. Where this raffle is communicated and/or conducted via a social media platform (including, but not limited to, Facebook, LinkedIn and/or Twitter), entrants acknowledge that use of social media platforms are subject to their terms and conditions of use. The raffle is in no way sponsored, endorsed, administered, nor associated with any social media platform.
